If possible, print out and complete the Patient Information Sheet. Bring it along with your insurance card and ID to your first appointment.
How to Make an Appointment:
Call the Office! Appointments for the Arlington Heights private practice are handled at 847-686-5700.
The receptionist is present in the office on Monday, Thursday and Friday.
When calling the office at high call volume times, you may get our voice mail. Please leave a message. Our staff will get back to you as quickly as possible.
When You Arrive at the Office:
Please sign in at the window. Complete the Patient Information Sheet if you are new to the doctor or have seen him only at the hospital. Have a seat – we’ll be right with you!
Billing:
Dr. Kut accepts and will bill directly most commercial insurance companies. If you have a co-pay or balance, this is expected at the time of the visit. You will, of course, be billed for any remaining balance. Credit cards are accepted.
It is suggested that you call your insurance company prior to making an appointment to confirm that Dr. Kut is in your plan.
Dr. Kut does not accept Medicare, Medicaid (Public Aid), any HMOs or workmans’ comp cases. Payment for services is ultimately patient/guarantor responsibility.

Refills:
You will always be given the correct number of refills to last until your next appointment. In an emergency, you can call the office Monday – Friday for a partial supply of medication to last until your next scheduled appointment. It is not possible to get prescriptions refilled on weekends due to staffing and access to your medical record. Don’t wait until you are on your last pill or two to call!
